I used Stampendous Aged Embossing Enamel (fun stuff!) on the left side of the tag. The right side is a piece of crumbled tissue paper that I colored with Distress Stains. The super cool broken glass is Mirror Paper!! Cool enough in the package, but when you peel off the protective layer, its like a real mirror!!
The skeleton heads are cut from grunge paper, painted and inked.
I used deco scissors (flashback!!) to cut the sign.
