Kindergarten class book for teacher...HELP!

I'm having a total brain fart...I'm making a scrapbook for my daughters Kindergarten teacher, and my plan was to email each parent a small list of questions that they could answer in their own words about why they like Mrs. T, what they like about school, etc...but I'm trying to come up with better ways to put the questions.

For any teachers, what questions do you think would be cute to see answered from kids in your class?

The book I picked out is an 8x8...and each kid will have their own page with their pic, then I'm going to put a pocket behind the pic with these questions in them. I was thinking we'd give the book at Christmas, or would end of school year be better? DD goes to a catholic school, so the religion/holiday thing isn't an issue about giving it as a Christmas gift..

I'm trying to think of ways I can get Mrs. T to take an individual pic with each kid, without her catching on to whats going on.

Not sure I can help with the questions, but if you're looking for a way to do photos I made a mini for a Sunday school teacher, she always put s Christmas tree up in her room so the night of the Christmas program I took her picture with each of her kids for the parents and kept a copy of each for her album. I had the album done apart from the photos and some embellies so I could finish it quick once I had the photos. Then at the Christmas party the next day I gave her the completed album.

I did one of these for my dd's 2nd grade teacher (she's a HS senior now) for the end of the school year. I just had them wrote one thing they loved about their school year.

Other questions could include:

what is one thing you learned this year?
what is your favorite subject? why?
what is your Christmas wish for Mrs. T (if you give it at Christmas)?
what is your favorite thing to do at recess?

It's a wonderful gift to give a teacher ... dd's teacher loved hers.

As far as the pictures of the teacher and student, how big is the class? I'm sure many parents took pics of their child with the teacher on the first day of school or meet the teacher night? Could they email you a copy of it? That way it wouldn't be so obvious.

The class is 13 students including my daughter. On the first day of school most parents just took a photo outside of the classroom door. I thought about that already. We went to a pumpkin patch today and I thought I might be able to get a quick pic with each kid but time was short.

My son's 4th grade class did this last year and it was adorable. Such a cute idea.

When my kids were in pre-school, the staff appreciation committee would provide forms to the parents with questions for us to ask our children. It included an area for us to attach pictures to as well.

The questions they asked were:
Miss Smith's class was so much fun because...

The Best thing I learned from Miss Smith was...

I think Miss Smith is special because...

In both situations, these were done as an end of year gift.
